I'm having the same problem. Generally happens after changing branches. Using 15.9.1. Tabs show up as AccountController:1 and AccountController:2

Same here. It all began after updating to 15.9.1. Basically, it happens when you checkout a different branch, that leads to a project reload (we're using Git btw). Here's an example: First, open a single tab with e.g. Task class (from System.Threading.Tasks). Now every time the branch is switched, the number of Task.cs tabs doubles, going from a single tab to 2 tabs, 4 tabs, 8 tabs, 16... and so on.
